15. 2d Lieutenant, 11th Kentucky Cavalry, is hereby dsicharged the service of the United States, to date February 17th, 1864, on account of physical disability, there being no rolls of his regiment on file since that date, and he having failed to file the necessary Surgeon's Certificates, as required by the regulations of the War Department.
