I, Charles T. Foulks, duly appointed by the Governor of the State of Kentucky, a Commissioner for the State of Kentucky, resident at No. 115 So. Seventh Street, in the City of Philadelphia, and State of Pennsylvalnia, do solemnly swear that I will well and faithfully execute and perform, all the duties of such Commissioner, under and by virtue of the laws of said State of Kentucky.
